Dose | Regular dose: 1 to 1.5g BID Adults: 500 mg BID or 1g once daily. Children- 15 mg/kg body weight TID (max. 2g daily). |

Precautions | 1. Avoid rapid infusion 2. Rotate infusion sites 3. Renal impairment 4. Elderly 5. History of deafness-avoid 6. The plasma-vancomycin concentration measured after 3 or 4 doses |
