Available dosage forms1. TABLETS
2. INJECTION
3. CAPSULE
DoseRegular dose: 1 to 1.5g BID
Adults: 500 mg BID or 1g once daily.
Children- 15 mg/kg body weight TID
(max. 2g daily).
Contraindications Allergy to corn/corn products,
hypersensitivity.
Precautions1. Avoid rapid infusion
2. Rotate infusion sites
3. Renal impairment
4. Elderly
5. History of deafness-avoid
6. The plasma-vancomycin concentration
measured after 3 or 4 doses
Adverse Effects1. Nephrotoxicity including renal failure and
interstitial nephritis
2. Ototoxicity
3. Blood disorders
4. Nausea
5. Chills
6. Fever
7. Eosinophilia
8. Anaphylaxis
9. Rashes
